*PLEASE NOTE ANY STUDENT WITH A MEDICAL CONDITION MUST INFORM THE SCHOOL NURSE*

Students with Asthma, Food or other Allergens, Seizure Disorders or any other medical conditions must have a written plan with medical orders signed by their Physician and Parent.  This needs to be updated yearly. 

Please be advised we are require the medication consent form for any medication such as; Tylenol, Ibuprofen, and prescribed medications by the Doctor if needed during school hours.  Prescription medication must be brought in the original container.  This information will need to be updated yearly. 

Please print out the two page "medication administration form" below and return with both pages completed.
